The Associate Patient Care Coordinator is responsible for staffing all new patient admissions/referrals, scheduling of daily patient visits for clinicians, review, and submission of end of day schedules, ensuring completion of all weekend schedules, and reviewing daily schedules with field clinicians. Supports the Clinical Team Manager (CTM) with maintaining daily contact with field clinicians and effectively communicating schedule changes to field clinicians. Critical thinking is essential for reviewing weekly active patient census reports, scheduling patient discharge visits with appropriate field clinicians, scheduling patient re-certification visits, supervisory visits with the primary registered nurse, and appropriate job tasks for field clinicians per physician orders. Regularly communicates with Intake Department personnel in reference to all new patient referrals and new physician orders for staffing with the appropriate field clinician. Assists with the review and processing of patient discharge and therapy evaluations. Collaborates with the Director of Nursing, Clinical Team Manager and Director of Home Health Operations to ensure all patients are staffed accordingly per physician orders. Actively participates in the Agency's on-call rotation in the Home Health Team Coordinator role, approximately once per month to include holidays.
Schedule: Monday- Friday from 8:00 AM to 5:00 PM or 9 AM - 6 PM CST - Alternating
